How to Fill Out & Upload the Pre-Authorization Form:

Click on the form link above.
Select “Download linked file.”
Open your device’s Files or Downloads folder.
Tap the downloaded form to open it.
Choose “Fill & Sign” or “Auto-fill form” (depends on your device/app).
Fill in the blanks and tap Done or Save.
Return to this page and tap “Upload File.”
Select your completed form — it will display once uploaded.

Tip: Some mobile devices require a PDF reader app to fill in the form.

If you are unable to fill out or upload the pre-authorization form, you can fill out a physical copy at the storage yard upon arrival, however, you will be unable to leave your items at the facility until the form is completed.
